Harbor Medical, Inc. was founded in 1994 by Carol Sears, RN, BS, CCM, CDMS. Ms. Sears has more than 30 years experience in Case Management Services and the Medical Field. She has served on numerous committees and boards including, Founding Member of the Palm Coast Association of Rehabilitation Nurses, Board Member and Treasurer for Florida State Association of Rehabilitation Nurses, Member of International Association of Providers in the Private Sector, Florida Hand Association and has been a member of Certified Case Managers since 1986 and Certified Disability Management Services since 1987. She provides the training, mentoring, and facilitation for all RN's / Independent Nurse Case Managers while also providing quality control on all files.

The company was started to provide Nurse Case Management Services targeted at the insurance industries needs to resolve difficult and challenging cases in a timely and financially responsible manner.
What we do!
Nurse Case Management Services require a proactive approach in which a highly skilled registered nurse collaborates closely with the patient/family, health care providers, and payer to indentify and achieve functional and timely outcomes. This is done by aligning the health care resources and maximizing communication, efficiency, and effectiveness. This includes the process of:
- Assessment
- Planning
- Implementation
- Monitoring
- Evaluation
Independent Nurse case manager qualifications:
All of our RN's / Independent Nurse Case Managers are licensed Registered Nurses (RN's) and have at least one current certification in: Certified Case Manager (CCM), Certified Disability Management Specialist (CDMS), Certified Occupational Health Nurse (COHN), or Certified Registered Rehabilitation Nurse (CRRN). All are fully insured. Our Independent Nurse Case Managers come from diverse backgrounds, this along with their extensive experiences allow them to address numerous issues, such as potentially hazardous places or situations. In addition they can also help identify any pre-existing pathology which may be preventing the recovery from the current illness or injury.
